Wrap a cloth measuring tape around your head just above your eyebrows and ears. Pull the tape comfortably snug, read the length, repeat for good measure and use the largest measurement. A piece of string will work too, then just hold it up to a ruler and measure.

"Hat looks good and is out together nicely - BUT. . . I have about a dozen Scally caps from this company - Most are the 'M/L' (medium/large) size. A few are 'S' (small) Literally NONE of them is the same size. Some 'M/L' are smaller than the 'S' size. I had exchanged those for an XL that was HUGE - Ended up returning the hat entirely - And out some money, in the end. I gave up thinking that exchanges are a viable option - And customer service is not helpful withe their canned responses. On THIS (M/L) hat - I was able to get my wife to disassemble the band and take it in about 3/8" - Now it fits. Meanwhile - I have other M/L that are very snug. . . And others that I hope I can get my wife to adjust, now that she came up with a solution. I buy the hats because I do like the styles - And I do have some that ARE a perfect fit. But - The sizing is wildly inconsistent, and customer service is no help no matter how many times I have explained this issue to them (usually ignored)."
